Once logged into the support portal, to create a new request you click on the Submit Request near the to right of the screen where you name now appears in place of Sign In.
This will take you to the request form where you can fill out the fields and submit the ticket.
After submitting the ticket, you will see the following view, where you can add new comments and see the status of the ticket.
